   • Do not email, fax, or upload digital copies, only original paper forms are valid.

Under Florida law, a public-sector union must maintain a minimum membership percentage to remain certified.
Completing and submitting your DOR form ensures that:

• Your voice continues to be represented in contract negotiations.

• Your workplace rights remain protected under the law.

• CWA Local 3106 stays an active and recognized bargaining agent for City of Jacksonville employees.
