Field Name | What to Include |

Your Name | First and last name (this must match your name on company records). |
Job Title | Your current job title (e.g., Service Technician, Customer Rep). |
Work Location | Be specific – include the building address or office name if known. |
Supervisor’s Name | The direct supervisor responsible at the time of the issue. |
Date of Incident | The exact date the issue occurred. If it’s ongoing, list the start date. |
Article Violated (if known) | If you know what part of the contract applies, list it (e.g., Article 21 - Grievance Procedure). If you don’t know, don’t worry – we’ll help fill this section out for you. |
Detailed Description of the Issue | Be clear and factual. What happened? Who was involved? What was said or done? What impact did it have? Include dates, names, and any supporting info. |
What Remedy Are You Seeking? | What would fix this? (Examples: back pay, schedule correction, removal of discipline, etc.) |
Your Contact Info | Include a phone number and/or email you check regularly. We will follow up. |
